3. Insert the specification as in the register.
4. Insert the full name and address of the registered user.
5. Insert the goods inrespect, which the registered user is registered.
6. State the manner in which the entry should be varied.
7. Signature of registered proprietor or of his agent.
8. State the name of the place of the appropriate office of the Trade Marks Registry-See rule 4.
